What is the "EOI?": This means "evidence of insurability". To clarify...

EOI is an application process in which you provide medical information regarding the condition of your health. EOI may be required to enroll in, add dependents to or increase some state insurance coverage.

This EOI is "Factual Information" used by insurance companies to determine an applicant's qualification for insurance. Examples of information used may include paramedical exams, medical records, application statements, and motor vehicle reports among others.
